New Patient Registration
Joining the practice is a simple matter of completing a registration form available from the reception desk. If you need help with this form our receptionists will be happy to assist you. Once completed, your details are registered on the practice computer system which then automatically requests the Health Authority to ask for your medical notes from your previous surgery.
If you are on regular repeat medication, please arrange to see a doctor with the repeat counterfoil (right-hand side) from your previous practice (before you are likely to run out), in order that this can be set up on our system, causing you the least possible disruption.
New patients are invited to complete a health status questionnaire which enables the practice to obtain basic information about your current state of health on joining us. This form. along with the registration form can also be downloaded from the above "Registering Online" tab.
The Health Authority no longer send medical registration cards unless specifically requested by the patient.
If you are a Carer or you have Carers looking after you, please inform the receptionist.
Changes of Personal Details
If you change your name, address or telephone number please complete a form available from the reception desk. Please ensure your details are always kept up to date and if the change affects other members of your family registered with the practice – for example, when moving house.
You may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
